Home health is a short-term treatment plan for patients who are homebound. Instead of going to a rehab facility, care comes to the patient in his or her own home. Home health is physician-managed therapy with the goal of improving the patient’s condition. It is is the ideal transition between hospitalization and independent living. There are earlier hospital discharges and lower health care costs.

Often patients who are referred for home health services need help with rehabilitation activities, wound care, IV antibiotics, and building strength after a procedure or accident. Family can be active participants in the health care and healing process. All services are coordinated with your physician and you have 24-hour access to a skilled Nurse.

How Do We Begin Care?

To begin receiving home health care from Golden Bird Home Health Care, Inc. you may phone us directly or call your physician, hospital, clinic, social agency, family member or friend to refer you. Once we have received your referral, a registered nurse will make an appointment with you at your earliest convenience to determine your qualifications for home health care. Once you qualify, we will work closely with your personal physician to develop your health care plan.
